Life Transformation Counseling partners with Therapy Appointment to provide secure online appointment scheduling. 

 

CURRENT clients: Use the Therapy Appointment link below to check availability and schedule an appointment with your counselor. You will then be directed to your Patient Portal to sign in and schedule. If you have forgotten your username or password please contact the office.

NEW clients: Please call our office at (813) 575-8199, if we are unavailable, you may leave a confidential message and we will return your call within one business day. Please be aware that our identifying information may appear on your Caller ID.

Before your first appointment

Prior to your first appointment, please complete all intake forms sent to you through THERAPY APPOINTMENT. If you are unable to complete the forms in advance, please arrive 30-minutes early to your initial appointment to complete them in the office.

For minor clients: Both parents must sign the consent for treatment unless legal documentation states otherwise. Please provide applicable court documentation prior to the first appointment.
